You can simply copy and paste this questionnaire into an email and type in the answers after each question. Please do not send as an attached file. Please answer ALL questions. I have a 30-day return policy on my dogs. If you decide the dog is not right for you and your home, you are welcome to bring him back unharmed within 30 days of the adoption for a full refund of the price of the dog. You can read more about this on the page titled "Puppies".

If you live outside the Southeastern U.S., please let us know before filling out the application. We have had several successful cross-country adoptions, but whether or not we will consider it depends on many factors.

Suggested references are friend, neighbors, supervisor at work, colleague. Dog trainers, groomers, or other animal professionals are especially helpful. We will call these references as part of the adoption process.

By submitting this application, you agree to cooperate with the adoption process. You acknowledge and agree that at any point in the process, we may decide that you are not a good match for the dog you are applying for and that no reason will be given. Often we have more than one highly qualified family willing to adopt the same dog. We love to adopt our dogs to new homes, but it is our responsibility alone to decide who will adopt them.
